The goal of the OCALI Services and Supports Database is to include all providers who serve individuals with disabilities in Ohio. The database is not complete. We encourage all service providers to submit their information so they can be included here. If you would like your agency to be included in the database, please download the service provider form and mail it to OCALI at the address listed on the form.
